Where and when were you born?

A birth certificate is the best source for the time. Memory is usually out by more than you would think.

Name at birth

Numerology is calculated from the letters of your name, so the full name you were given at birth gives the truest reading. Middle names change the numbers, so include them if you have them. It is also how the report addresses you.

Day, month, year, as on a birth certificate.

Sex at birth optional

Used for one thing: the direction the Four Pillars luck pillars run, which the tradition sets from this and the year of birth. Leave it and that part of the chart stays unread.
